Mon-Fri 8:00A.M.-6:00 P.M. Except Tues 8:00A.M.-8:00P.M.
Clinic personnel includes a Doctor, Nurse Practitioner, Nurses, and a Secretary. The Physician visits once weekly only. Appointments are highly recommended. Please notify the clinic of any cancellations.

Student must present a valid photo I.D. validated for the current semester in order to be treated at the clinic

All students treated in the clinic must complete documentation on the initial visit and sign a consent form to be treated

Medical records are legal documents and clinical consultations are confidential. To release medical records to yourself or an outside agency a written consent must be given by student
