Vendor Registration Information

Thank you for your interest in becoming a Florida Memorial University vendor. Before you begin the online registration process, please review the minimum information requirements to complete your registration.

  • Company Information
    Must provide company name, company overview, tax ID and federal tax classification
  • Conflict of Interest Disclosure
    Must disclose any potential for conflict of interest
  • Order Information
    Must provide the name of a primary contact, title, phone/fax number, street address, email address and preferred order receiving method
  • Payment/Remit To Information
    Must provide contact name, phone/fax number, street address, email address and upload completed, applicable tax form (W-8BEN, W9)
  • Minority Status
    Must be able to provide minority classification, certifying agency’s name and upload the certified minority certificate with this application

Please note that completing the vendor registration process does not constitute a relationship with, nor does it guarantee any future business with Florida Memorial University. It will allow the Office of Purchasing and Procurement Services to understand your sourcing capabilities, products and services for future procurement opportunities.
